You’ve spent years building the life you love. And as you age, you want to maintain your usual pace. Via Christi HOPE can provide you the care you need to stay active, independent and in the place you want to be: your home.
Launched in 2002, Via Christi HOPE is a local, comprehensive program of healthcare and support services based on the Program of All-Inclusive Care for the Elderly (PACE). To join, you must be age 55 and up, and be a resident of Sedgwick County, Kansas.
Care is provided by a multidisciplinary team who want to get to know you, your health and the life you want to live. Once they know your lifestyle and goals, they’ll work together to create a care plan that will help you remain in your own home for as long as possible.
Your care team includes:
- Primary care physician
- Registered nurse
- Social worker
- Physical therapist
- Occupational therapist
- Activity coordinator
- Day center manager
- Home care coordinator
- Personal care attendant
- Transportation representative
