Living with pulmonary or cardiac disease can be challenging. With proper care and lifestyle training, you can learn to manage your disease in your own home — allowing you a better quality of life and less risk of future ER and hospital visits.
Via Christi Community Cares helps make that possible. As a team of caregivers, we work with your physician to offer comprehensive pulmonary care. We bring our team to you, create an individualized plan and then provide your care in the comfort of your own home.
Your care will be led by nurse practitioners, who work alongside a team of specialists that may include:
- Registered nurses
- Respiratory therapists
- Home medical specialists
- Pharmacists
- Home care aides
- Social workers
All of our Community Cares specialists are trained to provide comprehensive healthcare services in partnership with your physician and family. Our goal is to help you achieve your highest level of health.
Our nurse practitioners and other team members will be with you and your family throughout your disease process. We are available to serve you 24 hours a day, seven days a week.
Any time that you transition into the hospital or other healthcare setting, a seamless hand-off will occur, so that your healthcare goals remain at the center of the plan. We look forward to building a relationship with you and your family.
No person should go without needed care based on their ability to pay. Via Christi's trained financial counselors are available to discuss your individual needs. We will review your eligibility for financial assistance and help you make payment arrangements. Counselors are available Monday through Friday, 8 am-4 pm by calling 316-268-5178 and choosing option 2.
